The Incident and Injury Report Form on the first page is simple to follow and should be completed by everyone who witnessed the incident.
The Incident and Injury Investigation Form (on the reverse side) should be completed by the person investigating the incident and signed off by management.
The company 'Incident Register' (not supplied) must also include details of the incident and reviewed on a monthly basis.
The two forms also introduce a basic risk level assessment regarding the seriousness of the incident and level of investigation that will be necessary as part of follow-up.
