Hi there I’m looking for a booklet/pad for any injuries or accidents that my employees have to sign etc
You will need an incident and investigation report form.
The staff fill out how they injured themselves, where they were injured and treatment that took place and any immediate steps taken to correct the action.
A supervisor needs to sign the form and investigate to see if any controls can be implemented to stop another injury.
Alternatively you could offer you staff a Workers Booklet. This has an minor incident report in it, as well as a Pre-start work assessment and emergency procedures.
