Take 5 Workers Health & Safety Booklet
The Take 5 Workers Booklet is designed to assist workers in assessing hazardous tasks prior to undertaking them.
The booklet includes sections on quick risk assessment, risk matrix, job safety analysis (JSA), minor incident reports and emergency information.
At 10cm (width) x 14cm (height) the booklet is easy to keep in your toolbox / vehicle glove box.
PRODUCT INFO
The Take 5 booklet begins with the Take 5 rules, asking the user to:
- Identify the hazard
- Think about what could go wrong
- Control the hazard
- Proceed with the task
Pre-Task Checklist
The fold out form requires the user to answer 17 yes / no questions to determine whether permission is required before they commence with the task.
Following the flow chart, positioned above the checklist, if all answers fall into a green box the user is good to begin the task, if one or more answers are in a red box they must complete a JSA on the back of checklist or complete a work permit or refer to the supervisor before beginning the task.
If the residual risk rating is low they may continue with the task. If the residual risk is moderate or greater they must contact their supervisor for review and obtain authorisation to proceed.

RETURN & REFUND POLICY
Lost Goods or Damages
Items are sent out in original good condition, packaged appropriately. Claims for any damage or loss in transit must be made as against the shipping carrier.
Returns Policy
Any items with faulty manufacturing must be returned within 7 days. The customer can return an item within 7 days of receipt of such item for refund or replacement. Customised products are exempt from this policy. Whether to accept the return of an item is at the complete discretion of Take 5.
SHIPPING INFO
All items are sent Nationwide from Christchurch. Orders are sent following business day order has been placed.
Shipping to North and South Island is $8.50, shipping to rural areas is $10.00. Orders over $150 receive free shipping. Shipping can take between 3 - 5 business days.
Pick up option from our Christchurch office is available.
